Access Care Plan

Over  the last year,our PCN practices have continued  to improve how patients access care by strenghening telephone systems,expanding online consultations services ,and improving care navigations process across the network. Practices have used patients feedback and access data to better understand demand and adapt serivces accordingly,supporting more timely responses and improved access to care. There has also been a continued focus  on supporting continuity of care  for patients with more complex or ongoing health needs.

Our ongoing access approach is based on providing fair,responsive,and easy-to navigate care through a combination of online telephone,face -to -face access routes. Practices will continue  to develop triage and care navigation systems,while listening to patients feedback through surveys,participation groups,and community engagement to help shape future improvements.

NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.
